Session Description:

Despite growing use, many pharmacists and providers lack the knowledge and experience to use buprenorphine properly for pain management. The goal of this session is to increase familiarity and comfort with the basics of using buprenorphine for pain management. Session attendees will gain a better understanding of how buprenorphine can be used for pain, its place in therapy, proper patient selection, and how to initiate and titrate buprenorphine safely.

  • Describe buprenorphine’s pharmacology related to pain management.

Title: Acute Pain Treatment for Patients Taking Buprenorphine

Session Description:

Clinicians are more frequently facing challenges managing acute pain in patients taking buprenorphine for pain or opioid use disorder. The goal of this session is to improve pain management for patients using buprenorphine products by increasing understanding of strategies to treat acute pain and improve outcomes during care transitions. The speakers present in-depth information on pharmacokinetic properties of buprenorphine, including comparison of receptor occupancy to other opioids, and use simulated patient cases to demonstrate how to approach the use of buprenorphine along with full agonist opioids for acute pain. The speakers address knowledge gaps on perioperative buprenorphine management and provide strategies for addressing buprenorphine when patients transition to end-of-life care.

At the completion of this activity, participants will be able to:

  • Define the challenges associated with treating acute pain with other opioids in patients taking buprenorphine. 

  • Compare strategies for managing acute pain in patients on low- and high-doses of buprenorphine. 

  • Summarize considerations for managing buprenorphine in the perioperative setting and transitions of care, including hospice and end-of-life.

Title: Buprenorphine for Pain in Patients with Tolerance to Full-Agonist Opioids

Session Description:

Off-label use of moderate- and high-dose buprenorphine products for pain management is growing, yet many clinicians are uncomfortable with the logistics of employing a buprenorphine micro-initiation technique to rotate patients tolerant to full-agonist opioids. The goal of this session is to promote a better understanding of buprenorphine micro-initiation for pain management. The speaker reviews different micro-initiation techniques and considerations for using different buprenorphine dosage forms to perform the micro-initiation. Simulated patient cases are used to demonstrate strategies for micro-initiation in a variety of clinical settings and scenarios. 

Learning Objectives:

At the completion of this activity, participants will be able to:

  • Recognize the differences in mu-opioid receptor binding affinity among common full agonist opioids and buprenorphine and their importance in micro-initiation. 

  • Review the different formulations of buprenorphine, their role in micro-initiation, and limitations of each. 

  • Compare different micro-initiation techniques and review of published literature.
